Contract for Phone and Email Consultations

Phone and e-mail consultations as a support to Nutrition therapy have been documented as effective components of the change process.

Outlined below are the terms for phone and e-mail consultation with Kate Zager, MS RD LD

Make initial contact with Kate through website or phone@ 630-484-5283.
After making the phone appointment, the client is responsible for making the call to Kate Zager at the time designated.
E-mails will be initiated by the client on the designated date. Kate will respond within 48 hours of receipt of the first e-mail.
Client is responsible for making payment of $50 (for 25 minutes) or $90 (for 50 minutes) in advance of the phone consultation. Discounts for payments made in advance and/or for multiple sessions.
Client is responsible for making payment of $50 for four reciprocal (8) e-mails.

A receipt will be returned to the client, by mail or e-mail reflecting the payment.
Cancellations must be made 24 hours in advance or the full fee will be charged. This fee is not covered by insurance companies
E-mails: Communication with Kate is available by e-mail. For every four e-mail exchanges a fee of $50 is charged. Reduced rates are available when payment is made in advance.
